HIPAA Policy The City of Mountain View is required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to establish City policy and procedures to protect the privacy of covered Personal Health Information (PHI) and to provide you with a notice of the City's legal duties and PHI privacy practices. The activities subject to HIPAA are the City's emergency medical response and self-funded health plans. This notice describes your legal rights; how PHI can be used and disclosed; and how an individual may obtain access, modify or limit access to PHI.
