Establish a Connection

• Recycled Water Use Permit Application
• Application for Water Service
• Standard Notes and Details for On-Site Recycled Water Systems
• Cross-Connection Control Certification Form
• Coverage Test Form

Maintain a Connection

• Annual Self-Inspection Report (due June 1 each calendar year)
